Complimentary CEU Luncheon - High Risk Care Transitions

Right at Home Atlanta is co-sponsoring a complimentary luncheon and continuing education unit (CEU) presentation open to employees of Northside Hospital, including social workers, case managers and registered nurses.  The  presentation will review how to transition high risk patients to the appropriate care setting, as well as interventions and best practices for meeting needs of complex patients.
