Hospital to Home Services in Inland Valley

After a stay at the hospital or other care facility, there’s nothing like returning to the comfort of home. A discharge from the hospital can seem like a welcome end to an injury or illness. But the transition to living at home can be daunting. It’s difficult to manage appointments, medications, and the routines of daily living such as grocery shopping and housework when all you want to do is rest and recuperate. That’s why we developed our own hospital to home program called RightTransitions® .

Through our RightTransitions hospital to home care program, Right at Home Inland Valley can help seniors and adults with disabilities get home safely and prevent further hospitalizations with regular visits by our trained caregivers. We’re here to make living at home easier, so you can save your energy for recovery.

How Can RightTransitions® Help With Your Hospital to Home Transition?

Right at Home Inland Valley can make transitioning from the hospital to home easy. Your care team will communicate with your family and health care providers to notify them of any changes in condition. This is important so that any small change being experienced as you recover can be dealt with quickly to avoid any more serious complications.

When developing your personalized care plan, we take into account your unique situation and health concerns, including the plan of care you received upon discharge, when offering hospital to home care services such as:

"I can’t say enough for the folks at Right at Home when I needed the services it provided most. They were the only agency to indicate immediate availability in providing care when my husband left the hospital and needed in home care right away. I could not have managed without Michael, Greg and William for the two weeks they provided care for my husband and assistance to me. Each of them, in their own way, established a bond and trust relationship with my husband. Even though their time with him was unfortunately short. "...." "I would recommend this agency without equivocation to anyone seeking peace of mind and assistance with their loved ones who are not able to care for themselves."

"Recently I was in need of assistance going to St Mary’s Emergency Hospital. I made the right decision and called Right at Home. Based on my experience last September, immediately I was placed in the capable, competent, and caring hands of Charles. From start to finish some six hours, Charlie was constantly at my side, anticipating every need, and accommodating in every instance. I know could not have managed without him. I am more grateful than I am able to express. His calm, steady and knowledge presence was necessary and made my situation a pleasant experience as it could be. Charles is an “Old World Gentleman “ the type of person so very rare to see in today’s society. Such a person needs to be celebrated and I write this letter as a testimonial to him and Right at Home"
