Hospital to Home Services in Portsmouth

After a stay at the hospital or other care facility, there’s nothing like returning to the comfort of home. A discharge from the hospital can seem like a welcome end to an injury or illness. But the transition to living at home can be daunting. It’s difficult to manage appointments, medications, and the routines of daily living such as grocery shopping and housework when all you want to do is rest and recuperate. That’s why we developed our own hospital to home program called RightTransitions® .

Through our RightTransitions hospital to home care program, Right at Home Seacoast New Hampshire can help seniors and adults with disabilities get home safely and prevent further hospitalizations with regular visits by our trained caregivers. We’re here to make living at home easier, so you can save your energy for recovery.

How Can RightTransitions® Help With Your Hospital to Home Transition?

Right at Home [location] can make transitioning from the hospital to home easy. Your care team will communicate with your family and health care providers to notify them of any changes in condition. This is important so that any small change being experienced as you recover can be dealt with quickly to avoid any more serious complications.

When developing your personalized care plan, we take into account your unique situation and health concerns, including the plan of care you received upon discharge, when offering hospital to home care services such as:

"Our care provider, Felicia, returned after her maternity leave and has been a wonderful aide to my Mom, who is 100 yrs old and in her own assisted living apartment. Felicia is very patient and anticipates my Mom's needs. It is a great comfort knowing my Mom has a caring companion to start her day!"

"We have been very blessed to have Marilyn come into our home every day. She provides my mom with lots of love and care. She allows mom to maintain her dignity while taking care of her daily needs that she once could do on her own. As a working daughter, I don't have to worry about my mom while I am away. I know that Marilyn has it all under control. I am always a phone call away, however, I can teach my students without worrying about Mom's welfare. We have always been very fortunate with our health care providers from your company. Keep up the good work and kudos to Marilyn! We love her!!"
