Hospital to Home Services in Inland Empire Rancho

After a stay at the hospital or other care facility, there’s nothing like returning to the comfort of home. A discharge from the hospital can seem like a welcome end to an injury or illness. But the transition to living at home can be daunting. It’s difficult to manage appointments, medications, and the routines of daily living such as grocery shopping and housework when all you want to do is rest and recuperate. That’s why we developed our own hospital to home program called RightTransitions® .

Through our RightTransitions hospital to home care program, Right at Home Inland Empire Rancho can help seniors and adults with disabilities get home safely and prevent further hospitalizations with regular visits by our trained caregivers. We’re here to make living at home easier, so you can save your energy for recovery.

How Can RightTransitions® Help With Your Hospital to Home Transition?

Right at Home Inland Empire Rancho can make transitioning from the hospital to home easy. Your care team will communicate with your family and health care providers to notify them of any changes in condition. This is important so that any small change being experienced as you recover can be dealt with quickly to avoid any more serious complications.

When developing your personalized care plan, we take into account your unique situation and health concerns, including the plan of care you received upon discharge, when offering hospital to home care services such as:

"They come on time and are very good. They follow up and call me. They are a great company. I don't worry about going to the store and doing my laundry. I am very satisfied. She helps me with my shower. She does my hair. She goes to the drug store for me. She does anything I ask her to do well. She helps me with my shower. She does my hair. She is just great because she really helps me and is always willing to come if I need her."

"My husband passed away last week, and both of us were receiving care. They're wonderful and very supportive. Even with my husband gone, I need their help. They stayed with my husband when my sister-in-law took me to the doctor. If it wasn't for them, I couldn't have cared for him for as long as I did. They took out the trash and did things that I couldn't do, like vacuuming and making the bed. They gave Mike food and were supportive of my needs too. Crystal is a character. She's always there when I call, and she immediately jumps in to help. They're very supportive and caring. They treated us like family."

"We saw them on a website that was advertised on TV. They were local, and they sounded good. We decided to try them. They are readily available, and I can call them anytime. They are usually pretty responsive, and the people they provide do a pretty good job. It is just a small part of my mom's life, but they are a part that is important. They take my mom to her hair and nail appointments, and they take her to activities for us. They are responsive and get back to me pretty quickly. They do the best they can to make things happen that I need. They seem to meet my mom's needs. I don't hear any complaints from the place she lives, since my mom has dementia."
