Medication Management Services!

Offered by The City of Albuquerque and Bernalillo County Area Agency on Aging

This free service is available to seniors (age 60+) in Albuquerque who qualify for in-home services and would benefit from the temporary assistance of medication management. 

During scheduled home visits, a licensed registered nurse can provide the following Medication Management Services: 

Medication Reconciliation

• Review medications for possible duplication (e.g., generic and trade name medications)
• Review medications for consistency with current physician recommendations
• Review medications for possible interactions

Consumer Education

• Explain the purpose of each prescribed medication
• Review the dosage and show how to read labels
• Caution about possible interactions with food or other medications
• Explain guidelines, restrictions, and side effects
• Inform about when to call the doctor

Medication Aides

• Assist with organizing medications
• Help with medication records and worksheets
• Inform about packaging options
• Offer suggestions about pharmacies that deliver
