After a stay at the hospital or other care facility, there’s nothing like returning to the comfort of home. A discharge from the hospital can seem like a welcome end to an injury or illness. But the transition to living at home can be daunting. It’s difficult to manage appointments, medications, and the routines of daily living such as grocery shopping and housework when all you want to do is rest and recuperate. That’s why we developed our own hospital to home program called RightTransitions® .

Right at Home Brings an Additional Level of Care Not Available from Other Agencies
First, we take advantage of our proprietary Change in Condition program to ensure that each of our trained caregivers knows exactly what to monitor for and how to respond when the patient exhibits a change that may traditionally result in re-admittance. Second, we are level III Residential Service Agency. This means that, in addition to help with Activities of Daily Living (ADLs), we also assist with medication administration and management and provide skilled care. Third, we are the only in-home care agency that worked directly with the Harvard School of Medicine and Forsyth Medical Center to establish the connection between in-home care and re-hospitalization avoidance.
We currently run a program with two major local hospitals in Anne Arundel County in an effort to avoid re-hospitalizations.
