Hospital to Home Services in Westborough

After a stay at the hospital or other care facility, there’s nothing like returning to the comfort of home. A discharge from the hospital can seem like a welcome end to an injury or illness. But the transition to living at home can be daunting. It’s difficult to manage appointments, medications, and the routines of daily living such as grocery shopping and housework when all you want to do is rest and recuperate. That’s why we developed our own hospital to home program called RightTransitions® .

Through our RightTransitions hospital to home care program, Right at Home Eastern Worcester County can help seniors and adults with disabilities get home safely and prevent further hospitalizations with regular visits by our trained caregivers. We’re here to make living at home easier, so you can save your energy for recovery.

This is important so that any small change being experienced as you recover can be dealt with quickly to avoid any more serious complications.

When developing your personalized care plan, we take into account your unique situation and health concerns, including the plan of care you received upon discharge, when offering hospital to home care services such as:

"Lilion and I have a good relationship and she is very trustworthy. My health is not completely obvious as most of my inabilities are heart related. My heart does not circulate blood making me slow to move and someday I do a lot of sitting or very easy on my morning. Lilion's help has done a lot for me being able to do something that was I was not able to keep up."

"Highly responsive team, very caring and involved owner; very competent nursing staff; quality of caregivers is good overall, and some are especially excellent. Office deftly handled LTC financials. RAH helped us locate and work with a fine Hospice agency. We were with RAH for three years with an almost consistently excellent experience, and when not, there were quick resolutions. Have had other experiences with providers; very few match up to RAH. Truly excellent. Highly recommend them."
