Per Diem Nursing Supervisor Job Description

  • Responsible for overall supervision and evaluation of health care services delivered.
  • Directs, supervises, trains and evaluates performance of assigned field staff (Certified Home Health Aides).
  • Helps maintain Home Care compliance with all standards and requirements of regulatory, accreditation, reimbursement agencies and contractors.
  • Maintains and enhances high quality clinical practice in Home Health Aide Care, including Quality Assurance Programs.
  • Supervises and supports Certified Home Health Aides, to ensure the highest quality and reliability of staff.
  • Collaborates with other Right at Home staff in conducting educational programs for seniors, caregivers and professionals.
  • Collaborates with other Right at Home staff (e.g., social workers, case managers and outreach staff) in conducting patient assessments, care planning and clinical programs.
  • $50-$75 per hour

Qualifications and Skills

  • Registered Nurse with a current New Jersey license in good standing.
  • Minimum of three (3) years experience in a clinical setting working with the aged.
  • Computer and phone skills are necessary.
  • Teaching experience preferred.
  • Licensed and insured driver with automobile.

Essential Functions

  • Receives newly assigned patient referrals and admissions for development and review of level of care, plan of treatment, nursing care plan, and scheduled assignment of field staff by the schedulers.
  • Performs initial patient assessments to assess patient status and direct the field staff to perform required care. Exercises discretion and independent judgment in planning for care of the patient.
  • Initiates patient care plans through patient assessment, condition and environment, in accordance with plan of care, and consultation with other health team members.
  • Establishes and monitors patient clinical records used in the home and in the office. Documents changes in patient status.
  • Performs written patient reassessment per policy or more as warranted. Follows up with patient/family regarding any changes to patient’s status or care plan.
  • Supervises field per policy to ensure the delivery of care as defined by their job description and patient needs. Completes an in-home supervision and evaluation of field staff at this time per Right at Home and the Home Care Department’s policy and procedures.
  • Supervises the delivery of patient care as defined by the field staff’s job description and the patient’s needs through review of records and reports documenting patient care. These are to be reviewed and signed when the Certified Homemaker/Home Health Aide has comments.
  • Communicates with the Home Care team regarding changes in patient status, and exercises discretion and judgment in resolution of these issues.
  • Enters and maintains, with the assistance of others, all patient and staff data in the Right at Home Management Information System. Responsible for oversight of data quality and completeness, with the Home Care Director of Nursing.
  • Coordinates patient’s care with other Right at Home staff, other providers, personal physician (as appropriate), contracts, or other related services to provide continuity of care.
  • Provides and documents clinical competencies of the field staff.
  • Assists in training or any activities required for certification or re-certification of field staff.
  • Performs occasional on-call duties as a Registered Nurse.
  • Performs other duties as may be requested by the Director of Nursing/Home Care and Administrator.
  • Acts as Director of Nursing/Home Care Services in his/her absence.
  • Assists to audits clinical records to ensure compliance.
