Seniors On the Move Shuttle

A valuable service for individuals living at home or in a facility without the ability to transport themselves.

If you need transportation to doctor’s appointments, medical procedures, assistance with shopping and other errands, visit with family and friends or just a day away from home —  Seniors on the Move can assist you.

 

We Understand The Scenarios of Life 

• Transportation can include the passenger and friend, relative or caregiver, maximum three people.
• Passengers must be ambulatory and able to get in and out of a vehicle with minimal assistance.
• Right at Home can provide a caregiver for additional assistance and a wheelchair or walker may accompany the passenger.
• Errand service available for those who can’t make the trip.

Service Rates & Hours

(Rates subject to change without notice)

►  TRANSPORTATION: Mileage is calculated from the origin of the passenger pickup to the drop off location. 
►  RATE: $2.00 per mile (10 mile minimum = $20 minimum for all transports) Rate discounted for distances exceeding 50 miles per trip.
►  WAIT TIME: $20.00 per hour (billed in 15 minute increments)
►  SERVICE AREA: All of Allen County (outside of Allen County, call for rate) Passenger must be ambulatory but can require the use of a wheelchair or walker. Passenger must be able to get in and out of the vehicle with minimal assistance. The wheelchair or walker can be transported but are not provided by Right at Home. 
►  IN-HOME AND IN-FACILITY CARE: As a provider of nonmedical care we also provide companionship, homemaking, and personal care services.  Please request Agency profile for services and rate information.
►  PERSONAL ERRAND SERVICE: Rate is $20.00 per hour with a two hour minimum. More than one errand can be combined into the minimum two hour period. Cost of purchases and third party fees are not included in the rate. Extended time packages available. Services rates include a maximum of three individuals per vehicle including a caregiver and or friends or family members.
►  HOURS: Hours of transport availability are 24/7/365. 

Payment: We accept cash, checks, and all major credit cards. Authorization signed at start of assignment. *Payment due at time of service.

Schedule Changes, Cancellations, No Show, Inclement Weather

►  Schedule Changes: Please schedule all transportation and any changes in route at least 24 hours in advance if possible.
►  Cancellation: There are no charges providing cancellation is within three hours of start. 
►  No Show: For the client’s safety and as a courtesy, it is important that we are notified of a cancellation or change in schedule. If there is a No Show our dispatcher will call clients contact numbers to verify that there is not an emergency situation. 
►  Inclement Weather:“Seniors on the Move” will not operate during inclement weather. We will call and advise clients of all cancellations due to weather.
