Hospital to Home Services in Marin

After a stay at the hospital or other care facility, there’s nothing like returning to the comfort of home. A discharge from the hospital can seem like a welcome end to an injury or illness. But the transition to living at home can be daunting. It’s difficult to manage appointments, medications, and the routines of daily living such as grocery shopping and housework when all you want to do is rest and recuperate. That’s why we developed our own hospital to home program called RightTransitions® .

Through our RightTransitions hospital to home care program, Right at Home Marin can help seniors and adults with disabilities get home safely and prevent further hospitalizations with regular visits by our trained caregivers. We’re here to make living at home easier, so you can save your energy for recovery.

How Can RightTransitions® Help With Your Hospital to Home Transition?

Right at Home Marin can make transitioning from the hospital to home easy. Your care team will communicate with your family and health care providers to notify them of any changes in condition. This is important so that any small change being experienced as you recover can be dealt with quickly to avoid any more serious complications.

When developing your personalized care plan, we take into account your unique situation and health concerns, including the plan of care you received upon discharge, when offering hospital to home care services such as:

"When I described the type of caregiver I need, they search through who they have to make the appropriate fit. They have made it so she is more independent. She has been able to live in the assisted living for longer than she would have been able to. They do physical transfers in and out of the wheelchair. They accompany her to activities. They reminisce with her. They are very communicative about who is coming. If there is a change in the schedule, they let me know. The owner will introduce the caregiver. They are very professional."

"They were very responsive and got back to me. If they have to change schedules or something, they will let us know. They provided company for her, which was very nice for her. They would get her to bed, change her, and just be a friend to her. They got back to me when I needed them to. The office was there for 24 hours a day, so I knew I could always get in contact with someone."

"They're very responsive and reacted very quickly. It was only one night, but it was needed right then and there. It was given right away. We needed a body to help him get to the restroom and watch him through the night. They were very responsive and came quickly. I looked for physical strength, knowledge, and empathy."
