Missing Link: Reducing Readmission Rates Continuing Education Event

Right at Home Skokie is proud to co-sponsor, along with Crystal Home Health, for a continuing education event, "The Missing Link: Reducing Readmission Rates." This event will be presented by Teri Dreher, RN, IRNPA, CCM of North Shore Patient Advocates. Health care providers, case managers and social workers are invited to join us for this informative presentation. You'll learn techniques for combining better post-acute care with improved communications for a positive impact on senior health following a hospital release. 

Refreshments and appetizers will be served. 1 hour credit hour provided by Colleen Morley.
