Change in Condition
At Right at Home, we know that a change in condition, no matter how slight, can cause big worry. That’s why change-in-condition monitoring is a part of all of our services. You can rest easy knowing that our caregivers proactively monitor for any signs of declining health.
How It Works
Our caregivers and office staff are trained to observe, report and act on any changes in your condition, particularly in the following five change-in-condition categories:

At the conclusion of each shift, Right at Home’s proprietary system requires our caregivers to report any changes in condition. If a change in condition is reported, it is automatically forwarded to our office staff. At that point, our office team communicates the change in condition to the care stakeholders, which could include:
With any change in condition, we coordinate with the family and relevant medical professionals to reassess your needs and revise your customized care plan as appropriate. Finally, our caregivers are trained to optimize their caregiving services under the revised care plan.
