AGE of Central Texas is hosting a free caregiver support seminar on “Ask the Experts: Hospital and Rehab Discharge Planning” on Thursday, November 21st. Part of the on-going workshop series to help family members effectively care for an older loved one, the free seminar will be held from 6:00 p.m. to 8:00 p.m. at the Austin Public Library, Hampton Branch at Oak Hill, located at 5125 Convict Hill Road in Austin.

Regional experts will discuss the importance of hospital and skilled nursing discharge planning to improve care management at home.
