Right at Home Cognitive Support Program - Results

Many of our clients are experiencing cognitive decline. As the decline occurs, whether due to dementia, Alzheimer’s or injury, the clients tend to withdraw and their family members often do not know how to adapt to their loved one’s new normal. To address these issues and improve the quality of life of those we serve, we launched the Right at Home Cognitive Support Program.  Below are some recent success stories and a brief overview of the program.

Right at Home Cognitive Support Program - Recent Success Stories

A lovely grandmother in her late 80’s lives with her son and daughter-in-law in their country home. She has advanced dementia and was spending most of the day sitting alone with minimal interaction as she pretends to read books and sleeps. As a result of the program she is now knitting, which she has not done in over 15 years, making beautiful gifts for her family. She is noticeably more alert and engaged and has also started taking walks and spending more time with others. Her family also better understands how to work with her and is at peace with her abilities.

Another lovely grandmother living in an independent living community has early stages of Alzheimer’s but saw significant decline when she was temporarily moved to a new apartment. She was forgetting her medications, the names of her children and was sitting alone most of the day. After just a few weeks, she is eating better and has gained weight. She is exercising each day and is noticeably more clear of mind. She is also playing the piano for the first time in many years and is engaging more with her children and neighbors.

These are just two of many cases where the Right at Home Cognitive Support Program has improved the quality of life of those involved. For the family member these were small miracles and wonderfully rewarding to us and the caregivers.

Right at Home Cognitive Support Program

Unlike traditional support programs, Right at Home’s approach to dementia and cognitive support targets the idea that ALL of our clients, regardless of the severity or progression of their cognitive change, have abilities to interact and connect with their surroundings. This breakthrough, research-proven approach ensures your patient or loved one gets the right care to remain living securely and independently at home, wherever home may be. The program has also shown to have great benefit to those living in memory care facilities who need added support to engage more fully in or integrate more successfully into their environment.

Keys to Our Approach

Ability: We create a care environment that empowers the client and their current abilities. This begins with a formal, research-proven assessment to determine ability levels and activities best suited for their ability levels.

Personhood: We incorporate the client’s life history, routines and preference into the Personal Plan. This plan is developed from visiting with the client and others that know and care for them using a detailed interview guide.

Lifestyle: We then collaborate with the client and family to set goals and create support activities in the areas of nutrition, socialization, wellness and awareness in order to minimize overall cognitive change and provide greater connection with their environment and loved ones.

Our RightCare cognitive support program is delivered by Right at Home certified Cognitive Specialist and certified Cognitive Support Caregivers.
