Right at Home believes that all persons have abilities regardless of their age or circumstance. Understanding a person’s abilities and meeting them where they are is fundamental to how we care for seniors. Thus assessments are key to our care. They help us, family members and even clients understand and accept ability levels, they establish benchmarks on which to measure our care, and they can act as motivators for clients to improve. Home Safety

Home Safety Assessment

Offered as a Public Service at No Cost

The Right at Home Safety Assessment goal is to keep you or your loved ones safe and reduce the likelihood of falls. Through this process our certified safety inspector uses a 100 point checklist to inspect each room and identify problem areas and recommend solutions. To help prevent falls, we have implemented the Right at Home Ask, Assess, and Act Program

We offer three highly reputable mobility and fall risk assessments that will give us a strong indication of a person’s fall risk.

  • The Timed Up and Go (TUG) Test
  • The 30 Second Chair Stand Test
  • The 4-Stage Balance Test

From the assessments, we develop specific action plans to improve mobility and reduce fall risk. These action plans might include the following:

  • RightCare Daily Movement Program
  • Multifactorial Risk Analysis (specially for clients who perform poorly in the assessments)
  • Client Education
  • Referral for physical, occupation or speech therapy
  • Nutrition enhancements including Vitamin D and/or calcium
  • Address foot problems and check footwear
  • Optimize vision
  • Optimize home safety
  • Medication management

Dementia and Cognitive Support Assessment


Right at Home has teamed up with Jackie Pool, the developer of the Pool Activity Level (PAL) assessment. The goal of this assessment is to discover the abilities, interests, and lifestyle of the person to find activities that bring them joy and to further engage them with their environment and loved ones.

Members of the Right at Home team are certified to conduct PAL Assessments.

The PAL Assessment Consists of:

  • Life history, interest and lifestyle (personhood) profile
  • Checklist describing the way a person engages in occupations
  • Activity profile with general information for engaging the person in a range of meaningful occupations

Individual action plan with activities in each of four areas:

  • Socialization
  • Nutrition
  • Wellness
  • Awareness

How to Get Started

The PAL assessment consists of visiting with and observing the client in their environment and interviewing those most familiar with the client’s personhood now and in the past.From there a formal assessment is completed and an action plan implemented and modified on a weekly basis over a period of 4-8 weeks.
