Hospital to Home Services in Plainview

After a stay at the hospital or other care facility, there’s nothing like returning to the comfort of home. A discharge from the hospital can seem like a welcome end to an injury or illness. But the transition to living at home can be daunting. It’s difficult to manage appointments, medications, and the routines of daily living such as grocery shopping and housework when all you want to do is rest and recuperate. That’s why we developed our own hospital to home program called RightTransitions® .

Through our RightTransitions hospital to home care program, Right at Home Nassau / Suffolk County can help seniors and adults with disabilities get home safely and prevent further hospitalizations with regular visits by our trained caregivers. We’re here to make living at home easier, so you can save your energy for recovery.

"I used Right at Home for about a month, and the staff was absolutely wonderful, compatible, and great. It’s a family-oriented business, and they were absolutely amazing. The scheduling was good; I worked overnight, so the accommodations to be able to talk to me at certain hours were actually wonderful. They were there when I needed them."

"Debbie called to thanking everyone for the amazing care her father received from us. She really appreciated Desmond and Mirchel. She said they were great, and the caregiver she appreciated the most was Yatmaram. He was so gentle with her Dad and clean him up so nice by giving him the best shave he has ever had … Debbie was very tearful and just wanted to let us know how much this meant to her and her family."
