Right at Home is thrilled to be the in-house provider for Park Plaza, a Jewish senior living community, in Chicago. Our mission is to Improve the Quality of Life for Those We Serve - our clients, caregivers, and community. Through this partnership, we are able to be in service to all three simultaneously, bringing our award-winning, Joint Commission accredited care model to the residents of this wonderful independent living community. We pride ourselves on being flexible and responsive to client needs, with no daily or weekly minimums or extended contracts. With Right at Home's shared care services at Park Plaza we are able to respond immediately to changes in care needs of residents, providing a safety net to support ongoing independence and autonomy.

Right at Home at Park Plaza

Right at Home is the exclusive provider of shared care services to residents at Park Plaza, a Jewish Senior Living Community, in Chicago. Shared Care aide service is currently available from 7am to 8:30 pm daily, but assistance for two or more consecutive hours is available through Right at Home 24 hours per day. A team of 5 nurses aides cover resident needs with as many as three on site simultaneously.

Shared Care

What is Shared Care?

Shared Care is a la carte services offered by the Right at Home team at Park Plaza to allow residents to receive care in increments of as little as 15 minutes or less at a time. These services are customized to the unique needs of the resident. Longer consecutive hour shifts are also available with coordination through the Care Management department at Right at Home.

Shared Care Services can include:

Bathing and Dressing assistance
Escorts to and from meals and/or activities
Laundry
Medication Reminders
Safety Checks
Emergency Response
Transportation
Garbage Removal
And more...
