GNYFC of NPACC on Readmission Prevention

The Greater New York / Fairfield County Connecticut (GNYFC) Chapter of the National Post-Acute Care Continuum (NPACC) had a networking breakfast and presentation today (Sept 27) at The Bristal at White Plains. Delegates of Right at Home from New York (Enrie Morales and Gregg Simons) and Connecticut (Tim Randall and David Babcock) attended the event to learn more about the best practices in readmission prevention in assisted living, skilled nursing and home care.

Mike Flair, Vice President of Franchise Business Solutions at Right at Home, gave a presentation on “Harvard at Home” – a joint research effort piloted in 2016 between Right at Home and the Harvard Medical School. Currently, 400 Right at Home locations in 45 states are participating in the program to report and monitor changes in conditions of elderly clients. The study aims at reducing preventable hospital readmissions and thus the healthcare costs.

Other speakers at the event include Albert L. Riddle, MS, CMD, HMDC, President and Chief Executive Officer of Riddle Medical, LCC, Medical Director/Consultant of Skilled Nursing Facilities; and Ronald Bucci, NHA, COS, RD, Senior Executive Director, The Greens at Cannondale, Wilton Meadows, The Greens at Greenwich.

The next GNYFC meeting will be held on December 6 at the Bristal White Plains.
