Greater New York/Fairfield County NPACC Chapter Meeting
In June 2018, the quarterly meeting for the Greater New York/Fairfield County (GNYFC) Chapter of the National Post-Acute Care Continuum (NPACC) was held at The Bristal at Armonk in White Plains, NY. The topic for the open to the public meeting and event was “Medication Complications: How to prevent medication errors leading to hospital re-admissions.” Speakers at the event, Dr. Paul Cavaluzzi, MD, Diane Corba, RPh and Joann Parliament, RN, discussed the role of the physician in the transition of care, the pharmacy's role in medication reconciliation and errors in medication from the VNS perspective.
Lou Giampa, owner of Right at Home Westchester, is a member of the GNYFC Chapter Board of Directors. The Chapter's purpose is the share best practices and education on post-acute care and care transitions through local and national events.
