Right at Home Westchester has partnered with Forefront Telecare’s nationwide provider network to connect clients in need of psychiatry &psychology treatment programs. Forefront is contracted with Medicare and most major insurance companies nationally. Forefront offers Mental Health Services in the convenience of your home for clients via Smartphones, Tablets, PCs or Macs!
Our Network of Psychiatrists, Psychologists, Psychiatric Nurse Practitioners and Licensed Clinical Social Workers have been serving senior populations in Long Term Care facilities and Hospital settings for over a decade is now available to serve seniors in their home.
Appointments for initial patient assessments can be scheduled during business hours. We work with each patient’s primary care provider to coordinate care across the care team. Forefront’s psychiatrists, psychiatric nurse practitioners, clinical psychologists, and clinical social workers stand ready to serve your needs.

Three Options For Clients To Participate:
For clients who express a desire to talk to a counselor, notify Forefront
A Primary Care provider can simply refer a client directly, notify Forefront
If unsure of client's particular care needs, Forefront's psychiatry practice provides an online screening for to help identify potential behavioral health issues
Program Goals
- Provide clients with support and access to professional resources.
- Identify & address behavioral health needs to improve quality of life.
- Match Forefront’s professional resources to each client's need, including Medication Management & Talk Therapy.
- Communicate and support clients needs in coordination with primary care providers.
Right at Home clients can request help with their state of mind or known conditions that have gone untreated by simply contacting Forefront to set up an initial assessment and determine a customized care plan for treatment.
If a primary care provider identifies or knows about existing behavioral health diagnoses, your provider may simply make a referral to Forefront for At Home care.
If a behavioral health diagnosis is not yet known, Forefront’s online survey tool can help uncover behavioral health needs such as depression or anxiety.
FastCheckNOW is an easy online screening survey that can be conducted by our home care team member. Identifying potential behavioral health issues and severity with an immediate report enabling the care team member to determine the level of severity and course of action warranted for a patient.

No Issues
No action needed.
Mild Issues
Care team asks client if they would like to see a provider, or notifies the PCP for a referral to Forefront.
Moderate or Worse
Care team notifies PCP of potential need for referral to Forefront.
