Ongoing Care Coordination

When Right at Home of Central New Jersey is working with a new client, there can be many issues to address and questions to be answered.

We approach all ongoing care coordination with the eye of a care manager. Once our client has returned home, our care coordination team gets to work. Our registered nurses oversee the work of the caregiver in the home to ensure that the proper care is being provided and that Right at Home of Central New Jersey is able to manage all of the client’s care needs. A registered nurse will create the initial care plan, call every 30 days and visit either in person or via a telehealth visit every 60 days throughout the entire time your loved one is under our care.

Our Director of Client Care along with our care coordination team continue to work with our client and their family as well! We are with you on your entire journey to recovery and our care coordination team will be with you every step of the way!

When our client’s Medicare Part A benefits run out, Right at Home of Central New Jersey ongoing care coordination program will coordinate our client’s Medicare Part B benefits with the appropriate provider. These Medicare Part B benefits cover services many of our clients continue to need and can benefit from and our Director of Client Care will work with the appropriate provider to arrange care as needed. Medicare Part B benefits can also be utilized without having had a hospital or rehabilitation stay and can be coordinated for our client as he/she ages in place at home.

All of the ongoing care coordination efforts above (and any more that might be needed in the future) are NEVER charged to our clients. Right at Home of Central New Jersey provides ongoing care coordination for FREE to our clients because we believe that morally and ethically “it’s the right thing to do” for every one of our clients so that they enjoy a better care experience. We believe in this because we always want to ensure that our client is having an overall excellent care experience in their home, above and beyond the fabulous home health aide level of care services that we pride ourselves on as well.

The ongoing care coordination provided to our client is monitored by our Right at Home of Central New Jersey Care Coordination Team, as well as our nursing staff.

We also have Executive CHHA Mentors who go to the client’s home as needed to provide additional training for our caregivers on an ongoing basis. They provide assistance to our caregivers by helping them as needed with tips on housekeeping, meal preparation, sanitary food handling, and cooking. Our Executive CHHA mentors can also educate our care staff on transferring our client (if required), cognitive and dementia support, bathing, bed bath techniques and fall prevention.

As mentioned earlier, our team of registered nurses checks in with the client and/or family every 30 days via a phone call and every 60 days via an in-person or telehealth visit to answer any questions, find out any changes in the client’s condition, and provide recommendations as needed. Of course, our nursing team is always available for any questions outside of their set visiting schedule.
