Free Event: Aging Well: Creating a plan and a team

Right at Home is a member of the Ventura County Homecare Association (VCHA), a local group of elder care companies that meet regularly to discuss care for our elderly. The VCHA is hosting an event featuring Dr. Jim Hornstein of Community Memorial Hospital about Aging Well: Creating a Plan and a Team. He will discuss Advanced Directives and Physician’s Order for Life Sustaining Treatment, also known as a POLST.
